ASHRM Monograph Reviews Data Analysis and Patient Safety
The American Society for Healthcare Risk Management (ASHRM) has released the third installment in its series of monographs on the use of data to promote patient safety. Data for Safety: Turning Lessons Learned into Actionable Knowledge recommends standardized processes for gathering and analyzing data regardless of how it is identified. To improve those processes, ASHRM recommends that hospitals consider steps such as involving multidisciplinary perspectives, using credible external sources for case studies and recommendations, exploring relationships with Patient Safety Organizations, providing incentives to executives and other managers such as patient safety officers for meeting patient safety goals, celebrating successes, and supporting a culture of safety.

CMS: Third year of value-based purchasing demonstration shows substantial improvement:
The latest results of the Premier Hospital Quality Incentive Demonstration show dramatic across-the-board improvement in the performance of participating hospitals, according to the Centers for Medicare & Medicaid Services. Launched in October 2003 by CMS and the Premier Inc. Healthcare Alliance, HQID involves about 250 hospitals in 36 states. The demonstration was designed to test new payment systems under Medicare that would improve the safety, quality and efficiency of care delivered in the nation's hospitals, according to a CMS release.

Harvard study: Mortality rates lower in Leapfrog hospitals
A study by the Leapfrog Group has found that hospitals that perform well on the Leapfrog Hospital Survey have lower mortality and better quality of care than those who either didn't perform as well on the survey, or who chose not to complete it. Additionally, the study, published in the Joint Commission Journal on Quality and Patient Safety, shows that hospitals that had begun to implement Leapfrog safety practices also had lower mortality rates.
